Arriving and leaving
Under Regulation 99 a child can only leave with a parent, an authorised person, or in accordance with written authorisation. These are the forms that give you that authorisation in writing.

Everyday consents and feedback
Sunscreen is the standing authorisation nearly every service collects. The complaint form exists because Regulation 168(2)(o) requires a complaints procedure.

Common questions
Do parents need an account to fill these in?
No. You send a link by email or SMS and they complete it on any device — no app, no password, no account. The signed copy comes back to your service and is stored against the child.
Can a form go to two parents?
Yes. Forms can have multiple signing steps, so a second parent or guardian can sign after the first, and every signature is timestamped against the record.
Which of these do we legally have to collect?
The enrolment record and its authorisations are required for every child under Regulations 160, 161 and 162. Excursion, transport and departure authorisations are required whenever those things happen. The rest depend on your service and your policies.
