Outside School Hours Care Enrolment Record
About the Outside School Hours Care Enrolment Record
A full digital enrolment record for school-age children in before school care, after school care and vacation care. It captures everything Regulation 160 requires an approved provider to keep — the child’s full name, date of birth, address and gender, parent and authorised nominee contact details, court orders and parenting plans, language and cultural background — plus the health information required by Regulation 162 and the medical treatment and ambulance authorisations required by Regulation 161. It adds what school-age services specifically need: the school and year group, the booking pattern for each session type, and written authorisation under Regulation 99 for a child to walk home, travel by public transport, or be escorted between the service and their school. Parents complete and sign it from a link on any device, and the nominated supervisor confirms the record and notes which documents were sighted.
